Medical, Dental, and Vision

Life Insurance
- Full-time and part-time with benefits employees eligible
- Benefit begins on the 1st of the month following 30 days of employment
- Basic Life and AD&D equal to one times base annual salary provided at no cost to full-time employees
- Supplemental life insurance for employee, spouse, and children available for purchase
Short Term and Long Term Disability
- Full-time and part-time with benefits employees eligible
- Benefit begins on the 1st of the month following 30 days of employment
Flexible Spending Accounts – Health Care and Dependent Care
- Pay for out-of-pocket health care and dependent care expenses with tax-free dollars
- Participate in either or both FSAs
- Contribute up to $5000 per year
- Receive reimbursement for expenses not fully covered by the medical, pharmacy, dental, or vision plans (deductibles, co-pays, hearing aids, over the counter drugs)
401(k) Retirement Plan
This benefit is available to employees after six months of service. Employees choose from several investment options and make contributions with pre-tax dollars, thus reducing their taxable income. As part of the discretionary match program, Capella can choose to match a specific portion of an employee’s salary deferral contributions. Matching contributions are typically funded once per year, after the close of the plan year.

Paid Time Off (PTO)
- Full-time and part-time with benefits employees eligible
- Accrual begins upon hire and is available to use after 90 days of employment
- Paid time off for vacations, personal use, holidays, and the first 40 consecutive hours of personal illness or injury. Any PTO remaining at the end of the year carries over to the next year.
- Full-time employees may accrue up to 22 days (176 hours) during their first through fifth year of service. PTO increases incrementally each year to a maximum of 32 days per year.
- Part-time employees accrue based on hours worked
Extended Illness Leave
- Full-time employees eligible
- Accrual begins upon hire and is available to use after six months of employment
- Use for paid time off for personal illness or injury lasting more than 40 consecutive hours
- Extended illness accrues at 3.692 hours per pay period up to a maximum of 600 hours

- Available to all employees
- Employee discount on purchases
- Meals may be charged and deducted from your paycheck
Parking
- Free, well-lit, fenced, security-patrolled and monitored

Employee Health Services
- Full-time on site RN
- Free and reduced cost immunizations (Influenza, Hep B, Hep A, etc.)

Child Development Center
EASTAR Health Services provides the benefit of on-site child care to its employees, and has offered this benefit since 1983. It is fully licensed by the Oklahoma Department of Human Services as a Two-Star Facility, and is licensed to accommodate 98 children at any given time. Services are provided for children age six weeks to six years old or prior to first grade. The operation of the facility is Monday thru Friday, 5:45 a.m., to 8:00 p.m. It allows parents to have a peace of mind while they’re at work to know their children are being cared for in a safe and nurturing environment. Care at EASTAR Health System has as its goal the nurturing of the whole child. Central to care is the presence of loving adults who give children as much personal attention as possible. Human interaction is valued in the Center. Continuity with family life is fostered through parent participation, family events, and times for conferring and sharing. The curriculum of the Center provides creative activities in an environment that lends itself to the special developmental needs of children as they grow, allowing them to experience all areas of learning, socially and intellectually.
